I really like Threadless.

There is just something about looking through the funky designs. I've definitely wasted time at this site and enjoyed it. (I really like scoring submissions.) While the items aren't exactly "handmade" they are circumventing the big box/big brand/boring offerings. I think an original t-shirt could be a great gift for my sister-in-law or her boyfriend or both of them. And gift certificates are always a good idea too.
